Community-based work, like everything in the 21st century, must embrace technological innovation. The Community Centered Family Health History Project, with 20 partners across the country, exemplifies the combination of traditional grassroots, door-to-door style efforts with web 2.0 presence. Through this presentation and those of two partners - Iona College and the Heredity Project - conference participants learned about the successful integration of family health history (FHH) information into existing programs and saw a demonstration of the new, customizable, online Does It Run In the Family? health history tool.
